Important Notes before you proceed with the application

I have read the Admission Guidelines and I understand that I am eligible to apply to the University of Macau.
I am aware that no request for change of programmes will be allowed after my submission of the online application for Doctoral Degree, Master’s Degree or Postgraduate Certificate Programmes.
I aware that the University of Macau reserves the right to withdraw any course, and in which case the fees paid will be refunded.
I agree that all documents submitted to University will be kept in my personal student file. I also agree that the personal data may be transferred within the University and to entitle that are in accordance with legal provision or with my prior consent, for the purpose of carrying out the related procedures.
I am aware that students whose studies have been discontinued due to unsatisfactory academic performance or who have exceeded their maximum study period cannot register as a student of the University within the subsequent year.
I acknowledge that I should not register in any other higher education programme in Macao in the same academic year. If such case happens, the University of Macau reserves the right to terminate my application or study at any time.
I ensure that the information provided on the application form and other documents submitted to the University for the application of admission to the postgraduate program is entirely true. I am aware that the University has the right to withdraw my application and/or terminate my studies at any time if any of the information given for the application of the program is later found to be untrue, false or incorrect or submitted in breach of the law.

Personal Data Authorization Page

The data in relation to your application will be used by the University of Macau to assess your suitability for pursuing postgraduate studies for which you are applying, and/or to determine the granting of scholarship or funding support. It may be transferred within the University and to entities that are in accordance with legal provision or with your prior consent, for the purpose of carrying out the related procedures.
The personal data of applicants may be circulated on the network without security measures and may be at risk of being seen and used by unauthorized third parties.
Completion of all items on the form is obligatory. Failure to provide the data may affect the processing and outcome of your application.
Please attach photocopies of ID/passport, academic diplomas and certificates, transcripts, together with your photo and any other documents as required.
To make correction of the personal data in relation to your application, you can submit the request to the Graduate School through the application website or email.
All information given in this form will be kept strictly confidential.

填寫網上表格前必須注意下列重要事項：

本人獲悉澳門大學之入學章程，並合乎資格報讀澳門大學之課程。
本人知悉於遞交博士學位，碩士學位或學位後證書課程網上報名表後，所填報之課程不得更改。
本人知悉澳門大學有權撤銷開辦任何課程。如大學撤銷開辦課程，報名費將予退還。
本人同意所有遞交之文件，澳門大學將記錄於本人的學生檔案中。本人亦同意個人資料可以在澳門大學內部及其他依法律規定或獲本人授權的實體之間傳遞，以進行相關程序處理。
本人知悉根據時效規定而喪失學籍的學生，從失去學籍之學年起，一學年內不得在澳門大學報名及註册。
本人知悉於同一學年內不得同時在本澳其它高等教育課程註冊。如有此等事件發生，澳門大學有權終止本人之學籍。
本人確保為報讀研究生課程所填寫及向本校遞交之資料全部屬實。本人亦知悉，倘若發現報讀研究生課程所提供之資料失實、虛假、不正確、或有違法律，澳門大學有權隨時撤銷本人之入讀申請及／或終止本人之學籍。

個人資料授權頁

關於閣下本次申請的相關資料，將會被澳門大學用作查核申請人所申請的研究生學位資格審查，及／或決定是否頒發獎學金或資助的用途。這些個人資訊亦可在澳門大學內部及其他依法律規定或獲閣下授權的實體之間傳遞，以進行相關程序處理。
申請人的個人資料在網絡上的流通可能缺乏安全保障，有被未經許可的第三者看到和使用的風險。
申請人必須完成填寫申請表上的所有項目。如申請人未能提供資料將可能影響申請人的申請及審核結果。
請附上身份證／護照、畢業證書及證明、成績單的複印本；並連同申請人的照片及所要求的相關文件。
如欲修改關於閣下本次申請的個人資料，申請人可通過申請網址或電郵向研究生院提出申請。
所有於本申請表上的資料將會以保密形式保存。
